Our providers include medical directors, SNFists (skilled nursing home care specialist doctors), and nurse practitioners, together creating seamless medical infrastructure management.

Mary Beth E. Sutkowski

The owner and chief medical officer of Paragon Clinical, Dr. Mary Beth Sutkowski, is a board-certified internist and native of the Chicago metropolitan area. She received her medical degree and completed her residency at Loyola University Stritch School of Medicine, where she was asked to stay an additional year as chief resident.

Dr. Sutkowski completed a two-year fellowship in internal medicine at Loyola University/Hines VA Hospitals, where she researched coronary risk prediction models and co-authored a study that was printed in the American Heart Journal in 1999. She worked 13 years at Advocate's Dreyer Medical clinic, pioneering their first Skilled Nursing Home Rounding Program. For more than a decade Dr. Sutkowski exclusively managed Humana Gold patients in skilled nursing facilities, a group for which Dreyer assumed full risk.

Dr. Sutkowski has held medical directorships in nursing homes and hospice groups and served on quality assurance and utilization management committees. As Paragon's medical director, she works with an outstanding team of caring and talented professionals who are driven by a passion for delivering quality health care to the elderly.

Dr. Sutkowski has helped develop medical infrastructures for skilled nursing facilities and assisted living residences in the Chicagoland area. She also works as a SNFist and is part of a telemedicine program that provides remote, after-hours clinical care in nursing homes. Dr. Sutkowski has a passion for providing state-of-the-art medical care and medical management systems for the elderly and disabled. 

Jennifer Edens, BSN, MSN, ACNP

Vice President of Clinical Services

Jennifer Edens

Jennifer Edens received both her Bachelor and Master Nursing degrees and completed her acute care nurse practitioner (ACNP) training from the University of Illinois at Chicago. She has more than 19 years’ experience as a nurse practitioner in pulmonary/critical care, cardiology, cardiovascular surgery, and long-term care.

In recent years, she was integral in starting the pulmonary hypertension and right heart failure program at Northwestern Memorial Hospital. She also managed a large pulmonary clinic at Northwestern and worked in cardiology at Central DuPage and Delnor Hospitals.

Jennifer loves geriatrics and continues to see patients while leading her nurse practitioner team at Paragon.

Bob Sutkowski

Vice President of Business Operations

Bob Sutkowski, our VP of Business Operations, brings over 30 years’ experience in new product development and program management. He has used his analytical approach to management to improve efficiency, ensure profitability and increase productivity contributing to Paragon’s growth. Prior to Paragon, Bob worked at Life Fitness in various technical and management roles including managing an engineering group.

Bob graduated from Illinois Institute of Technology with a Bachelor and Master degrees in electrical engineering. He also has completed graduate work in control theory and has training in the Lean Six Sigma methodology and project management.

Floy Haschke, MS, APN, CANP

Floy Haschke

A board-certified adult nurse practitioner (CANP), Floy Haschke earned her Bachelor of Science in Nursing from Northern Illinois University and her Master of Science in Nursing from Rush University. She has more than 25 years of experience as an adult nurse practitioner, in both inpatient post-acute settings and outpatient clinical settings, caring for adults with acute, complex, and chronic illnesses.

As director of quality for Paragon, she collaborates with the medical director, VP of clinical affairs, and the management team. She provides didactic and clinical orientations for new Paragon nurse practitioners, serving as preceptor and mentor. She functions as a consultant for company nurse practitioners. She manages, coordinates, and conducts chart audits for quality assurance. Her previous positions at Paragon included team leader and senior nurse practitioner.

Floy is involved in the NIU mentoring program for both undergraduate and graduate students. She is also active in NIU Alumni Association scholarship fundraising programs. Floy is a member of the Gerontological Advanced Practice Nurses Association, American Association of Nurse Practitioners, and Illinois Society for Advanced Practice Nurses.

Mary Ann Kabat, BSN, MSN

Clinical Nurse Educator

Mary Ann received her Bachelor’s Degree in Nursing from Loyola University of Chicago and her Master’s Degree in Nursing from Northwestern University. She has more than 20 years’ experience in rehabilitation nursing care, management and teaching. She was the clinical nurse specialist for the brain injury program and an educator in staff education and continuing education departments at the Rehabilitation Institute of Chicago. She started her own education and consulting business that developed and taught educational programs to prepare nurses for the Certified Rehabilitation Registered Nurse exam as well as provided educational and consultation services to rehabilitation units in acute care settings. 

As the clinical educator for Paragon, she provides Continuing Education in-services to long term care nursing staff on the care and management of patient issues.  She is driven by the conviction that effective education can affect outcomes for geriatric and disabled persons.  She is a member of Sigma Theta Tau, American Association of Nurse Practitioners, and the Association of Rehabilitation Nurses.  


Nelli Khallak, MSN, FNP-BC

Team Leader

Nelli Khallak, board-certified family nurse practitioner, earned both her Bachelor’s and Master’s degrees in Nursing at Purdue University. She has more than 10 years of nursing experience in a cardiovascular setting and four years of experience as a nurse practitioner in both inpatient and outpatient settings, caring for adult and geriatric populations with multiple acute and complex chronic illnesses. Her specialty areas include cardiology and diabetes management.

As a team leader, Nelli serves as a resource person for NPs in her region and aids with clinical and administrative issues in the buildings while collaborating with the management team. She enjoys caring for the geriatric population.

Alexandra Sulik, BSN, MSN, APN-BC

Team Leader

Alexandra Sulik is a board-certified adult nurse practitioner. She received both her Bachelor of Science in Nursing (2005) and Master of Science in Nursing (2010) degrees from Loyola University in Chicago. She has more than 14 years of healthcare experience, with seven of those years as an adult nurse practitioner.

Alexandra started her nursing career in orthopedic/medical-surgical nursing in 2005. Since becoming an NP in 2010, she has worked with geriatric and primary care patients to promote healthy lifestyles and manage chronic care conditions. She also is a co-owner of an adult daycare facility that specializes in non-medical dementia care and assists families in prolonging independent and safe living for their loved ones. Geriatric care has been a long-time interest and passion that has guided her in her healthcare career.

Michael Meaney, BSN, MSN, GNP

Team Leader

Mike Meaney is a Board Certified Gerontological Nurse Practitioner.  He received his Bachelor's of Science in Nursing from the University of Iowa and his Master's of Science in Nursing from the University of Illinois at Chicago.  He has over 18 years of experience in the healthcare industry, including hospital, outpatient clinic, and home health.  He has practiced as a nurse practitioner for the last seven years.

Mike has a special interest in geriatrics, and a fondness for working with older adults and their families.

Michelle Buikema, BSN, MSN, FNP-BC

Lead Palliative Care Nurse Practitioner

Michelle Buikema, a board-certified family nurse practitioner (FNP-BC), has over 12 years of health care experience and a passion for and expertise in palliative care. She established a palliative care program for Providence Hospice and is in the process of receiving palliative care certification and her doctorate of nursing practice at Wilkes University.

Michelle graduated with a Bachelor degree in Nursing from the University of Saint Francis, Fort Wayne in 2004. She worked at Providence in various nursing management positions and was involved in mentoring and educating nursing staff for over eight years.

Michelle completed her Master of Science in Nursing and family nurse practitioner certification at University of Saint Francis, Fort Wayne in 2011. She is a member of the Gerontological Advanced Practice Nurses Association, the American Association of Nurse Practitioners, Hospice and Palliative Nurses Association (HPNA), and American Academy of Hospice and Palliative Medicine (AAHPM).

Jennifer Johansen, BSN, MSN, APN

Team Leader

Jennifer Johansen is a Board Certified Adult Nurse Practitioner.  She received her Bachelor and Masters of Science in Nursing from the Loyola University.  With over 17 years of experience in the healthcare industry, including hospital Neuro ICU, skilled nursing facilities, and palliative care.  

She has practiced as a nurse practitioner for the last eight years.  Jennifer enjoys working with the geriatric population and coordinating care plans with families.

Lisa Landreth

Manager of Client and Employee Relations

Lisa Landreth has more than 15 years of experience in product development and operations in an international setting. She is a graduate of Illinois State University, with a bachelor’s degree in marketing and business administration. Lisa has volunteered for hospice, PADS (Public Action to Deliver Shelter), and several local non-profit organizations.

Gloria Werner

 Office Administrator

Gloria Werner comes to Paragon with over 12 years of experience as a business manager.  Her strong organizational skills, payroll experience, and administrative expertise are valuable assets to Paragon.

Our Providers

The nurse practitioners who work with Paragon all have advanced degrees and have been specifically trained to provide care to the geriatric population. They are compassionate and talented providers who take great pride in helping to manage both acute and chronic care situations and to encourage healthy lifestyles in every one of their patients.
